N2 - Summary. A multicentre retrospective observational study was conducted to analyse immunosuppressive regimens and outcomes by sex in incident patients who underwent kidney transplantation between 2009 and 2019. A total of 4,029 kidney recipients, 35.5% of whom were female, were considered. Immunosuppressive regimens were similar between male and female patients, with the most commonly prescribed regimen being tacrolimus + mycophenolate + steroids. Females had a higher risk of infection compared to males (HR = 1.33; 1.16-1.53), but no differences were observed for other outcomes. Further research is needed to understand the influence of sex and hormones on drug responses in order to tailor pharmacological treatment across different life stages.
