N2  -  Summary. We report the case of a 43-year-old woman who developed prolonged chest pain and syncope on September, 2019. A CT-scan was performed and spontaneous coronary dissection (SCAD) was diagnosed in the posterolateral ramus (PL) leading to a conservative management. Few days later, the patient entered again the Emergency Department with chest pain. Coronary angiogram confirmed the PL no-flow limiting dissection and a de-novo dissection of left descending coronary artery with thrombolysis in myocardial infarction (TIMI) 2-3 flow, and again medical therapy was suggested. A new recurrence of SCAD was observed two years later involving OM2 branch. Conservative management is preferred in most cases, proceeding to revascularization for patients with ongoing chest pain, hemodynamic instability and ST elevation.
