In this multicentre observational study, we investigated gender differences in long COVID and evaluated the predictive role of gender, age, comorbilities, and disease severity in the persistence of different long COVID symptoms in a cohort of 308 patients from the Ligurian region with previous symptomatic SARS-CoV-2 infection. Females showed higher prevalence of fatigue and problems in the quality of life (QoL) domains of usual activities, pain and anxiety/depression. The persistence of dyspnoea was associated with COVID-19 severity; the persistence of fatigue and impairments in the QoL domains of usual activities, pain, and anxiety/depression were associated with female gender; impairment in the mobility domain was associated with comorbidities. In conclusion, different factors are associated with distinct phenotypes of long COVID syndrome.
